The policy is not meant to constrain or hinder individuals' efforts to administer surveys to the University community *. However, the policy is aimed to ensure that surveys administered are well structured, organized and respect participants' privacy **. Well planned surveys and administration procedures are integral to the success of any evaluation or research project. |

Surveys are broadly defined as tools for collecting information from the University community through interviews or the application of questionnaires to a representative sample of community ***. |

Before a survey is administered to the University community it will be evaluated by University administrators and / or academic leaders. The approval process depends on the purpose of the surveys and the relationship between survey administrators and participants.
Survey administrators requiring official approval before administering surveys include:
- members of the University community seeking to evaluate opinions and behaviors of other University community members as part of academic research
- members of the University community seeking to evaluate opinions and behaviors of other University community members to inform and influence University policy
- external individuals/organizations seeking to collect information related to the AUD community or workings as part of a formal research project

Members of the University community (staff / students) as part of academic research
The following information and materials must be provided to the Institutional Effectiveness Director and the Chair / Dean of the academic department to which the research relates:
- Survey purpose and objectives
- Desired target population
- Means of data collection
- Ensuring that participants anonymity is respected
- Copy of the survey
- Copy of the consent form++
- Future use of collected information
The Institutional Effectiveness Director and the Chair / Dean of the respective department will review the presented information and contact surveying individuals with their final decision within 7 days of receiving their requests. |

Members of the University community (staff / students) as part of a research to inform and influence University policy
The following information and materials must be provided to the Institutional Effectiveness Director and the Provost for evaluation:
- Survey purpose and objectives
- Desired target population
- Means of data collection
- Ensuring that participants anonymity is respected
- Copy of the survey
- Copy of the consent form++
- Future use of collected information
The Institutional Effectiveness Director and the Provost will review the presented information and contact surveying individuals with their final decision within 7 days of receiving their requests. |

External individuals/organizations as part of a formal research project
The following information and materials must be provided to the Institutional Effectiveness Director, the President and Director of External Affairs:
- Survey purpose and objectives
- Desired target population
- Means of data collection
- Ensuring that participation is voluntary
- Ensuring that participants anonymity is respected
- Copy of the survey
- Agreement to provide a copy of final report to the University
The Institutional Effectiveness Director, the Provost and the Director of External Affairs would review the presented information and contact surveying individuals with their final decision within 7 days of receiving their requests. |

++ In the case of internal surveys, consent forms are only required if surveys administered will be published. |

| By accepting the authorization to administer the survey, individuals understand and acknowledge that they will have an obligation to use data complied responsibly, ensuring that data is not redistributed to parties inside or outside the University community, unless clearly mentioned when requesting survey approval. **** |

IE Calendar |
The Institutional Effectiveness Calendar illustrates dates of administration for pre-established surveys. It is advised that survey administrators review the calendar to ensure that their surveys do not conflict with other survey administration dates or critical events on campus. |
